Who pays

Medicare covers up to 100 days of skilled rehabilitation after a qualifying hospital stay. Long-term care is private pay until assets are spent down, after which Medicaid, the state Medicaid program, pays for most residents. Long-term care insurance may apply.

How to choose

Look at the health-inspection rating and the staffing rating before the overall rating, read the most recent inspection report, visit unannounced, and ask about nurse turnover. A facility flagged for abuse or in the Special Focus program deserves extra questions.
